Requirements:
Thank you for your interest! Please complete and submit an enrollment form via Procare.
Once submitted, we will get in touch with you. If a spot is currently not
available, you will be added to the waiting list. We will contact you as soon as something becomes available.
Required Forms:
1. Online Registration form via Procare
2. Hard copy of the registration form. This form is for OCFS compliance and will always remain on the premises.
3. Medical Form completed by your child’s physician.
4. 1-month deposit, which is non-refundable. If you provide us 2-months notice prior to leaving. In this case, the deposit will be applied as the last month’s payment. The deposit is calculated as four weeks of the rate for the hours you need.

We bill monthly. Our billing cycle is 4-5 weeks

We offer a 10% sibling discount

We charge $10 per 15 minutes for late pickups

Late payments of tuition are charged a $40 fee

Bounced check fee $30

Payments are due on the 10th of each month

We accept payment through Procare, Zelle, Quick Pay, cash or check
Waiting List Policy:
To get on our waiting list we require a one-time fee of $49 (which can be submitted through Quickpay/Zelle using our email- zusinfamilydaycare@gmail.com). Please note that it does not guarantee a spot, but you will have priority. If a spot opens up, the $49 will go toward your deposit.
